Valley Travel Associates Passenger Profile Form:  Please use this form if you are booking with us for the first time.  The form provides us with the details that we will need to customize your vacation booking and ensure that you receive all of the services that you require to make your vacation a Dream Vacation!  This is a general form that we use for all clients whether they are travelling on a cruise or a fully customized land itinerary.  If some portion of the form doesn’t make sense for the context of your vacation, feel free to skip that part of the form and continue.  This form also contains a section to convey credit card details.  You may elect to skip that section if we are in the initial planning stages of your trip.  If you do complete that section of the profile, we will hold the payment details on file.  Your credit card will not be charged until you provide explicit authorization to use the information on file for specific charges.  Please ensure that you complete the form indicating your name exactly as it appears on the Passport or RealID that you will be using for your air travel.  Incorrectly conveyed names may result in the denial of boarding on flights or cruises, or large change fees charges by airlines to correct misspellings or ID mismatches.

Valley Travel Associates Payment Authorization Form:  Please use this form to securely convey your credit card details to us to process deposits and other payments for your Dream Vacation booking.  Completing this form will not automatically process your payment--it will securely convey the credit card details to your travel advisor.  Payments are not complete until your travel advisors processes a payment with the travel supplier(s) used to assemble your specific itinerary.
